We look forward to providing you with a thorough eye evaluation using advanced technology and highly trained staff in a caring environment.
We are located at the Medical Center on James Street in Morristown. Your appointment will be in Suite 3E on the third floor unless the staff directs you to Suite 2D for specific testing. There is free parking, a handicap entrance and elevators for your convenience.
A comprehensive medical eye examination will require approximately one and a half to two hours. Vision testing, as well as dilating drops may be used to evaluate your eye health. The drops will cause blurry vision, the effects of which will last for several hours. Although many people are capable of driving themselves after the visit, some find it helpful to bring along a driver because of light sensitivity. Also, we advise that you bring sunglasses, as your eyes will be sensitive to bright light.
In order to ensure a prompt check in, please have the following items (if applicable):
- A referral from your primary care physician, if required by your insurance company.
- All prescription glasses and contact lenses
- Complete contact lens prescription
- Insurance card(s)
- A photo ID
- A list of all medications (including eye medications)
Forms
If we are out-of-network with your insurance carrier payment is due at the time of visit. However, we can submit your claim for reimbursement to your insurance on your behalf. If we participate with your insurance company, no payment is due at your visit unless you have a co-pay.
If you need to reschedule your appointment for any reason, simply call our office within 24 hours and we will gladly find a more convenient time for you.
There are several forms to be completed by new patients. You are welcome to print them in advance and bring the completed forms to your appointment. Please feel free to contact our office if you have any further questions. We look forward to seeing you in the near future.
